Student Support Parent/Guardian Consent Form
St Edward’s College is committed to providing quality education to its students. In an effort to achieve this goal, school staff or parents/guardians may refer students to one of our Student Support Services staff members. The aim of this service is to help students have a more effective education and socialisation within the school community. In today’s world, young people are bombarded by worries and stresses, therefore at St Edward’s College, we offer support for coping with changes, self-esteem, friendship and relationship issues, study skills, stress management, fears or worries, academic progress, conflict resolution, social skills, adjustment to school or culture, etc.
Because these sessions are based on a trusting relationship between the staff member and student, the staff member will keep information shared by the student confidential except in certain situations in which an ethical responsibility limits confidentiality. You will be notified under the following circumstances:
- The student reveals information about hurting himself/herself or another person.
- The student or another person may be in physical danger.
By signing this form, I give my informed consent for my child to participate in student support sessions as needed. I understand that anything that my child shares will be kept confidential except in the above-mentioned cases. You may revoke this consent at any time.
